How to Fix "Sum of G-J Must Equal M1-M6"

Your 300A case classification totals do not match your injury and illness type totals.

What this means

On OSHA Form 300A, total cases in columns G, H, I, and J should equal total injury and illness type counts in M1 through M6. If those two sides do not balance, the ITA may reject the upload.

Why this happens

  • One side of the 300A summary was updated, but the other side was not.
  • A case was counted in a case outcome column but not in an injury or illness type column.
  • Spreadsheet formulas were copied incorrectly before CSV export.
  • Zero-injury files include inconsistent nonzero totals.

How to fix it manually

  • Add total_deaths, total_dafw_cases, total_djtr_cases, and total_other_cases.
  • Add total_injuries, total_skin_disorders, total_respiratory_conditions, total_poisonings, total_hearing_loss, and total_other_illnesses.
  • Make sure both totals are equal for each establishment row.
  • If no injuries or illnesses occurred, confirm all case and type totals are zero.
